Under the forms section there is a new QVOA form for 2016 that has been added. Changes to the form include:

• being able to enter your school name under the city section if you wish to have your cheque mailed out through interoffice mail. This will save on postage and costs to QVOA.
• a second page that:
  • lists official's rates taken from the SHSAA Handbook
  • has a checklist to be completed before sending in your form
